The visiting parent or grandparent

One form per applicant. If both parents are coming, complete this once for each and bring both packages.

The visit

IRCC requires coverage valid for at least one full year from the date of entry, so the start date matters.

Medical history

This section decides whether a claim is paid. Most policies cover a pre-existing condition only if it has been stable for 90 or 180 days before coverage starts, and "stable" includes no change in medication or dosage. Be complete — a non-disclosure can void the whole policy, not just one claim.

Anything else

Anything the medical step did not have room for, travel dates you are unsure about, or a question for the meeting.

Your documents

The visiting parent or grandparent’s passport page, and anything recording their medications and recent medical history. These are read by your browser and bundled into the package you download. They are never uploaded.
